Description of Duties: The individual in this role will serve as a Case Manager of the GHV Programs. The GHVP Case Manager will work with the Individuals on continuing to keep lease agreements with current landlord and Individual on any issues or concerns regarding the lease agreement. The GHVP Case manager will be responsible for the completion of all documents before submission to Program manager, and DBHDD regional manager. The GHVP case manager will monitor and encourage continued Mental Health and Substance Abuse treatment which may include or is not limited to i.e. Non-Intensive outpatient or Intensive outpatient services. The GHVP Case Manager will complete Housing Survey in all agency programs and/ community agencies such as; Hospital, School, Jail, Probation/Parole. The focus will be to work towards whole health & wellness to maintain stability/ independence with everyone. They will work with individual on identifying barriers to treatment, provide support, stabilization and recovery. The GHVP Case Managers will effectively manage a caseload of up to 50 individuals, complete the minimum requirements of no less then (2) contacts per month with (1) being face to face contact or (1) collateral contact. Provide detailed progress notes of visits to home, community locations and phone contacts. All documentation should be completed within 24hours once service has been provided. They will be responsible for meeting the productivity standards of no less than 80% or higher each month. They will be responsible for completing plans of care and developing interventions that are person centered according to DBHDD guidelines. They will provide follow up on referral services as needed by completing necessary documentation to ensure each Individual is receiving all services within our agency and/or outside the agency. The GHVP Case manager will serve as liaison between Individuals, their physicians, other organizational components to ensure communication/ linkage and continuity of care of services for the Individual. Transporting individuals served is a requirement.
Minimum Qualifications: Bachelor’s Degree in psychology or related field. Must have a valid, unrestricted Georgia Driver’s License. Must be computer literate.
Preferred Qualifications: Three (3) years or more of experience working with individuals with Behavioral AND/OR Substance use Disorders.
