INTAKE COORDINATOR – BEHAVIORAL HEALTH

Horizon Health is seeking an Intake Coordinator to work on the inpatient 22-bed adult inpatient psychiatric unit and 14-bed substance abuse inpatient unit in the FirstHealth Moore Regional Medical Center in Pinehurst, NC.

The Intake Coordinator, in conjunction with the Program Director and Nursing Supervisor, coordinates the referral development and pre-admission patient assessment process, and provide clinical assessments for potential clients. This is a full-time position.

Some of this position’s responsibilities include but are not limited to:

• Conducts on site patient assessments in the Emergency Department, as needed
• Responds to intake calls and completes admission paperwork
• Coordinates treatment with physicians and other healthcare and social agencies
• Coordinates and participates as a direct liaison with appropriate interested parties and physicians through the patient referral inquiry and admission process
• Reviews newly admitted and recently discharged patient accounts

Requirements:
Requires Registered Nurse (RN), or Master’s prepared Social Worker or Counselor (Licensed MSW or Counselor preferred). Must have at least two years’ experience in admissions, intake or marketing social services; preferably behavioral health/psychiatry.
