St. Anne's Retirement Community has a full time position for an RN or LPN with MDS experience. 32 hours biweekly will be spent doing MDS with the remaining hours spent in Nursing.

Shift: 11:00 am - 7:00 pm

Must be available to work every other weekend and holiday. Must be either a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a current unencumbered PA state license. EOE

Job Specific Essential Functions

1. Demonstrates knowledge of the RAI manual.

2. Following the schedule provided by the RNAC, completes designated sections of the MDS following State and Federal regulations.

3. Completes data entry into the MDS computer system as needed.

4. Completes CAA summary sections as designated by the RNAC.

5. Reviews the MDS information for which responsible for accuracy before signing.

6. Creates and/or updates Care Plans as needed.

7. Designated MDS nurse attends Resident Care Conferences, progress meetings and reviews and discusses care planning information for each resident with the Interdisciplinary Team.

8. Assesses residents.

9. Performs the functions of the Registered Nurse Assessment Coordinator (RNAC) as needed.

10. Assists with Harrisburg Field Operations review team visits.

11. Prepares schedules following OBRA/PPS guidelines for initial, quarterly, significant changes, annual, PPS assessments and communicates schedule to IDT and sets up appropriate assessments.

12. Prepares Medicare letters for non-coverage and SNF Advance Beneficiary notices in accordance with CMS guidelines, attends progress meetings.

13. Reviews 24 hour reports and attends daily rounds to identify residents who may need updates to care plans or have a significant change in status. Receives information from the interdisciplinary team, determines if a significant change has occurred and plans for a comprehensive assessment.

14. Responds to the Quality Improvement Organization (QIO) of Pennsylvania representative when a Medicare appeal is requested and provides all necessary documentation.

15. Demonstrates knowledge of Medicare Part A for Skilled Nursing Facilities, including Advantage plans (PDPM vs RUGS).

16. Demonstrates a working knowledge of PA Case Mix/Medicaid for Skilled Nursing Facilities.

17. Demonstrates the ability to function independently in the MDS Nurse role, using critical thinking skills to complete the requirements of the job.
