We are currently seeking Physical Therapists and Speech Therapists for a Home Health Rehabilitation Department .

Type of Opportunity: Full Time

Work Schedule: Weekday Schedule Monday-Friday

Qualifications for Physical Therapist

Bachelor’s degree and educational requirements needed to be licensed as a Physical Therapist in New Mexico and to meet requirements of Home Health Medicare Conditions of Participation if necessary. One year experience in supervised clinical practice in acute care or rehab setting preferred. Basic typing competency preferred. Need to have current drivers license as well as auto insurance

Education:

* Bachelor’s degree

*Evaluates patients, develops plans and goals, and implements treatment programs for improvement of physical or developmental dysfunction and independence in activities of daily living. Continuously reassesses treatment plans and goals.

*Evaluates, observes and instructs clients on work related functional skills. Educates and instructs regarding home programs and preventive measures as appropriate to patient, family, and others.

*Initiates documentation in the home and completely records all care delivery as required by the profession and the department by the end of each day; this includes admissions.

*Communicates with patient, family, physician, and other healthcare providers regarding patient care.

*Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.

*Participates in educational programs and inservices and other meetings as required. Participates on special project teams and other related duties as assigned and requested.

*Performs other functions as required.

*Maintains a department defined productivity standard for visits and caseload management...

*Precepts newly hired therapists and students and serves as a resource person and professional role model as needed.

*Responsible for holiday and weekend coverage for agency and patients home visits as required on a rotating basis.
