Location Information
We have four senior communities in New Hope, Brooklyn Park, Shoreview, and Woodbury that are rich with thoughtful amenities and support. Our compassionate services also reach seniors living in the broader Twin Cities area through home care, outpatient therapies and wellness programs.

Responsibilities
The Clinical Informatics Specialist (CIS) will utilize clinical assessment skills, competencies, best practices along with new emerging technology to formulate holistic care plans that honor and uphold what matters most to the resident in conjunction with the STREAM grant. The use of innovative technology will allow for more accurate objective assessment data to be synchronized with current assessment data. The CIS and care team will then use this data, along with resident preference, to create individualized care planning strategies.
Understanding the significance that care planning along with resident needs has on quality of life will be a vital part of this role. The CIS will use their knowledge of unique late life care needs to support a balanced approach to care using maintenance and restorative care strategies with the goal of avoiding unnecessary decline while recognizing inevitable age-related decline.
Qualifications
- Registered Nurse with active Minnesota licensure.

- Proven passion, confidence, and candor required for late life conversations
- Must be competent and willing to educate others
- Willingness to utilize, participate in, adapt and realign work using the PDSA cycle (Plan, Do, Study, Act)
- Positively contribute to the collaborative consortium environment
- Willingness to follow direction of the Empira collaborative
- Proficient computer skills with Microsoft Office applications and other required software programs
- Must be able to function independently, demonstrate flexibility
- Ability to manage multiple tasks/projects and deadlines simultaneously
- Understand, interpret, and utilize data to improve resident care
- Ability to interact with all stakeholders from varying social, cultural and educational backgrounds
- Excellent interpersonal, communication, and organizational skills both written and verbal
- Experience with various office equipment
- Demonstrated ability to read, write, speak and understand the English language to communicate with all customers
- Desire for continuous growth and development
- Ability to have flexibility hours to accommodate all stakeholders involved in late life conversations
- Demonstrated compatibility with organization mission and vision as well as the STREAM Program
