Clinical Liaison (RN) - Admissions-Tyler hospital based

Full-Time: Mon-Fri 8am-5pm

Job Summary:

Performs assessment and admission of the patient to Hospice of East Texas

Job Qualifications:

• Current license as an RN in the State of Texas
• Documentation of at least two years or equivalent of nursing experience in an acute care setting
• Current CPR Certification as required by policy
• A minimum of (2) years hospice or home health experience preferred
• Demonstrates knowledge of Microsoft Office and ability to learn software programs specific to job functions
• Valid Driver’s License
• Proof of Current automobile Liability Insurance
• Demonstrate ability to relate effectively with physicians, health care professionals, the community, patients/families, and member of the hospice interdisciplinary team
• Employee must be FIT tested for an N95 mask

Physical Requirements:

• Frequently required to stand, walk, reach with hand and arms, stoop, kneel, twist, stretch, and crouch.
• Frequently required to walk and sit.
• Regularly required to use hands to handle and feel; and to speak and hear.
• Occasionally lift and/or move up to 50 pounds.
• Ability to deal effectively with stress
• Specific vision abilities required by this job include close, distance, color, peripheral, depth perception, and ability to focus.

Working Conditions:

• Patient home settings, nursing facility settings, general office settings, inpatient care facility, health care offices/hospitals

Working Relationships:

• All comments, actions and behaviors have a direct effect on the public's perception of Hospice of East Texas. Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be in a manner that is courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork and positive relations.
