Position Summary:

Assumes responsibility for providing social services to home health patients and families at home according to agency policies, applicable regulations and physician’s orders. Serves as a resource for other members of the home health team. Maintains acceptable visit productivity. Provides accurate documentation as required by regulations and agency policies.

Essential Functions:

Provides medical social services to home health patients and families under the direction of the patient’s physician or the Assistant Director.

Evaluates the psycho-social situation of the patient and family, and develops an individualized social plan based upon the social work evaluation and relevant input from other members of the home care team involved in the patient’s care.

Provides ongoing social services geared to restoring the patient’s optimum level of psycho-social and health adjustment. Such services should be based on the individualized plan developed for each patient.

Makes appropriate patient/family referrals to other community agencies to assist patients and families to obtain needed products and services.

Provides appropriate follow up phone calls or visits to patients and families and alters the individualized social work plan as indicated.

Promptly and accurately documents pertinent information in the patient’s medical record utilizing the designated agency forms and following agency documentation policies.

Consults with the Team Leaders/Case Managers at least daily to obtain new referrals and discuss updates regarding patient or referrals.

Reviews appropriate medical record documents to obtain pertinent patient/family information before initiating contact with newly referred patients and consults with the appropriate coordinator to obtain clarification regarding newly referred patients as needed.

Job Requirements

MSW from a school of social work accredited by the Council on Social Work Education.

Minimum of two years experience in a health care setting, preferably home health.

Licensed to work as a social worker in the State of Florida

Valid driver's license with an acceptable record
